Recognition of Prior Learning

INSTRUCTIONS

Complete each section. Failure to provide this information will delay the processing of your application. Normal enrolment fees are charged for Recognition of Prior Learning. You will need to enrol in the unit before your result can be registered. Completed forms can be submitted to the Student Management office in Warrnambool or the Reception Desk at any campus.

SECTION 1 – Personal data

SECTION 2 – Information about the unit in which you are seeking Recognition of Prior Learning and the evidence you are using as the basis for that Recognition of Prior Learning.

SECTION 3 – Supporting Documentation – All Recognition of Prior Learning applications must be accompanied by:

3.1 An original or a certified copy of your evidence.
3.2 The appropriate documentation if your name has changed since obtaining the qualification or experience provided as evidence.

SECTION 4 - Signature and date


Successful Recognition of Prior Learning results will be registered as the following:
